    Abstract
    Background The most important cause of hemorrhagic stroke is hypertension. The age with the highest risk of stroke is the elderly age 75 years. (Setiawan, 2020). From the results of data obtained in the ICU room at Bangil Hospital, it was found that CVA Bleeding patients who underwent craniotomy and experienced respiratory failure and were installed on a ventilator in February 2022 obtained 9.6% of the total 52 patients, in March 2022 it was 11.9% of the total patients. a total of 67 patients and in April 2022 obtained 25% of the total 40 patients. Stroke can trigger respiratory failure and can cause excessive sputum accumulation which causes ineffective airway clearance, especially in stroke after craniotomy and a ventilator is attached. Methods: Case study research. The research subjects used were 2 patients or 2 cases with the application of brochial hygiene Results: In both cases, the management experienced a buildup of sputum production in the airways. Analysis: The nursing diagnosis formulated is ineffective airway clearance related to neuromuscular dysfunction. Planning refers to IDHS, SLKI, SIKI. The interventions used were airway management and respiration monitoring. Airway suctioning and mechanical ventilation management. Implementation was given according to the planned intervention. The nursing evaluation used was SOAP with an evaluation of the suction/suction intervention for the two patients being managed, namely increased airway clearance. Resume: The intervention of suction and nebulizer is one of the effective interventions used to overcome the problem of ineffective airway clearance in hemorrhagic stroke patients
