Performance assessment, which this study focused on documentation, is implemented to evaluate nurses' performance, especially for completing the nurse care documentation. This documentation proceed is very important for nursing practice because it records many things in nursing practice. However, in reality, there are many nurses, whether they‘ve been worked for a long time either just worked in time, that still have less knowledge about nurse care documentation. The purpose of this study is to determine the relation of knowledge level and the length of nurses’ working with nurse care documentation performance in the Anwar Medika Hospital, Sidoarjo. The design of this study used nonexperimental: correlational. The population involved Anwar Medika Hospital’s nurses which total is 66 samples. The sample is taken by using the total sampling technique. The instruments used in this study were a questionnaire on the level of knowledge of nurses about nursing care documentation, a checklist sheet for the length of work of nurses, and an observation sheet for nurses' performance in documenting nursing care. Data were analyzed using the Crosstabulation Correlation Statistical Test. The results showed that the performance of nurses in documenting nursing care reached 96% (bad category). Of the 17 respondents with 6-10 years of work and sufficient knowledge, 39% had poor performance and 2.4% had good performance. The result of this study is that the better the level of knowledge and the longer the nurse's work, the better the nurse's performance in documenting nursing care. There is a relationship between the level of knowledge and length of work with the performance of nurses in documenting nursing care in the inpatient room of Anwar Medika Hospital, Sidoarjo. The implication of the research result is that it is hoped that the better the level of knowledge and the longer the nurse's work, the better the nurse's performance in documenting nursing care.en_US
