    Abstract
    Patients with CVA Infarct often experience problems in physical movement due to decreased muscle strength due to motor and sensory disturbances. If the problem is not treated immediately will result in muscle atrophy. The purpose of this study was to describe the implementation of post-hospital nursing care through home visits. The research method is a case study. Participants in this study consisted of two male post-CVA Infarct patients in Gondang District with the criteria for experiencing post-CVA Infarction, unable to move the extremities, stiff joints, physically weak and no complications. The implementation includes monitoring blood pressure before starting mobilization, involving the family to help the patient improve movement, evaluating the family's ability in range of motion exercises explaining the goals and procedures of mobilization, recommending early mobilization, teaching simple mobilizations to be done (sitting in a chair), demonstrating how exercise range of motion (how to train the range of motion on the pareseed side of the extremity using a normal extremity). The results of the 7x24 hour evaluation of the problems experienced by client 1 were partially resolved due to a longer history of CVA Infarction and outpatient treatment was not continued and influenced by age. Whereas in client 2, by giving the same nursing action, it can be resolved according to a predetermined plan and the intervention is maintained because the history of CVA Infarction is still not long and the age of the client is younger than client 1. Motor disorders in CVA Infarct patients if given early training then they can carry out optimal mobilization. It is expected that the family and health workers provide exercises to the extremities that experience weakness in CVA Infarct patients with impaired physical mobility problems.
