dc.description.abstractIntroduction: Prolanis is a health care system and a proactive approach that is held in an integrated manner involving participants from health facilities and BPJS health who suffer from chronic diseases to achieve an optimal quality of life with cost-effective and efficient health services. The purpose of this study was to explore the implementation of the Prolanis program in an effort to improve the health status of people with diabetes mellitus at the Kepohbaru Health Center, Bojonegoro Regency. This research design method uses a qualitative approach which is descriptive phenomenology. Based on the results of exploratory research on 7 participants, which was carried out through in-depth interview techniques. Results: There were 3 research themes, including: Input, Process, and Output of Prolanis at the Kepohbaru Health Center. The conclusion of this study indicates the importance of optimizing the implementation of the Prolanis program by providing continuous training on Prolanis to health workers at the Puskesmas; socialization of the program in the community and the synergy of empowering community resources to optimize the Prolanis program, so that the optimization of a good Prolanis implementation can improve efforts to improve the health status of people with diabetes mellitus in particular.en_US
