    Abstract
Damage to skin integrity is skin damage that occurs in the demis and/or epidermis, or tissue damage to the mucosa, cornea, fascia, muscle, tendon, bone, cartilage, joint capsule, and/or ligament. In patients with diabetes mellitus who are not treated and who do not control their diet (diet) can be at risk of causing complications of gangrene wounds. Gangrene wounds that occur often require patients to be hospitalized for a longer time so that the cost of treatment required will be even greater. The purpose of this study was to carry out nursing care for Diabetes Mellitus clients with Skin/Tissue Integrity Disorders at Masyithoh Bangil Hospital. The method used in this research is descriptive method with a case study approach. Nursing care was carried out from March 26, 2021 to April 22, 2021. The results of the study were obtained after 7 days of nursing care, it is hoped that skin integrity problems can be resolved, and the wound looks better. Within 7 days the wounds of both clients seemed to be improving because every day proper and sterile wound care was carried out, and assisted by the client's compliance in taking medications that had been prescribed by the doctor and the appropriate diet. In client 1 there was no purulent wound, there was no slough of red wound color, the wound was moist while on client 2 there was granulation tissue, no pus, pink wound color, moist wound. It is hoped that the hospital can provide optimal health services, especially for patients with Diabetes Mellitus.
