    Abstract
    Introduction Safety culture is an output of individual and group values, behaviors, competencies and habit patterns that reflect the commitment and skill style of the organization's management and patient safety. Medical error is a threat to every patient who wants to take care, therefore the importance of a patient safety culture is applied in every hospital to keep patients safer in terms of health services. This study aims to see the description of patient safety culture at Siti Hajar Islamic Hospital Sidoarjo. The type of research used is quantitative with a descriptive approach by describing each variable studied. With sampling using Random Sampling with a sample of 156 employees who work at Siti Hajar Islamic Hospital Sidoarjo. The results of the study show that in general the average patient safety culture is 83.73%, the dimensions of teamwork in the unit in the good category are 95.83%, the openness of communication in the good category is 81.62%, the hospital management support with a good category of 78.5%, teamwork between units in the medium category of 76.07%, adequate staff in the medium category of 70.19% and the perception of health workers on patient safety in the good category of 85.90%, My hope on the actions of superiors in promoting patient safety 86.06%, Feedback and communication about errors 96.58%, Non-punitive responses 74.08%, cooperation between units 88.62%, Handsoff and transition 79.29%, Overall perception of the patient safety 85.90%, overall perception of patient safety 71.79%. The conclusion in this study is that the safety culture at Siti Hajar Sidoarjo Islamic Hospital is categorized as good, meaning that health workers have implemented a safety culture but are optimal in carrying out health services according to SKP (patient safety standards) in the hospital. It is recommended that the hospital maintains, continues and develops patient safety programs, placing health workers according to Standard Operating Procedures. The role of the organization learning dimension of 98.72% in the 12 dimensions of patient safety culture shows that the hospital is actively carrying out program activities to improve patient safety, learn from mistakes to make changes for the better and evaluate their effectiveness. Employee ignorance and fear are also contributing factors for low incident reporting. This can be improved by motivating employees through understanding safety induction or making incident reporting an additional point for the Performance Index value.
