    Abstract
In Indonesia, currently the highest cause of maternal mortality is during the postpartum period, including from pregnancy, childbirth, and childbirth, which are not handled properly and on time. The author aims to apply the Continuity of Care method for postpartum mothers, neonates to family planning to detect early complications that can occur and prevent causes of death in mothers and babies. The author provides assistance to Mrs. F, aged 22 years by visiting during the postpartum period, neonates, and family planning through visits at home and via online. Care for Mrs. F was given from March 19, 2020 to April 27, 2020, as many as 4 postpartum visits, namely 2 home visits and 2 online visits, 3 times neonatal visits, 2 home visits 1 time online, and 2x family planning visits online. As a result of the postpartum visit, the mother did not have any complaints and her condition was good. At the first neonatal visit, the infant had no complaints. And on the first visit, the family planning informed the mother about the kinds of contraceptives for breastfeeding mothers and the second visit the mother agreed to use 3-month injection of contraception. He care that has been given can be understood and applied. Mothers are cooperative in examinations and receive good health education, so that the authors can provide care according to the needs of mothers and babies. With the continuous midwifery care during the postpartum period, neonates and family planning can early detect complications that occur so as to reduce MMR and IMR, as well as increase knowledge for mothers about postpartum and infant care.
