dc.description.abstract | The postpartum period, neonates, and family planning are quite important
periods for health workers to always carry out monitoring because less than optimal
implementation can cause mothers and babies to experience various problems, and
even lead to complications. Therefore the authors will carry out continuous
midwifery care (Continuity of Care) so that it can monitor and detect early
complications that may endanger the mother and baby. Continuity of care is carried
out by providing comprehensive and continuous midwifery care during the
postpartum period, neonates and family planning with the varney midwifery
management approach, and documented using SOAP. Midwifery care for Mrs. "M"
P20002 was carried out on March 15 2020 - April 25 2020 at Puskesmas Kutorejo,
Mojokerto Regency. The author conducted 4 postpartum visits, 3 times neonatal
visits, and 1 time family planning. During the postpartum visit, it is done once at the
health center and 3 times at the patient's house, the results of the physiological
examination. At the neonatal visit, one visit to the health center and two times at the
patient's house at the same time as the postnatal visit, the results are physiological.
The family planning visit is carried out once at the patient's house. During the family
planning visit, the mother is sure to choose to use the 3-month injection of
contraception. The results of the examination obtained during the postpartum period
of Mrs. "M" are within normal limits. By carrying out midwifery care for postpartum
mothers in a Continuity of Care, it is hoped that every mother who is in the
puerperium, neonates and family planning can pass this period without any
