    Abstract
    The puerperium is the period after delivery is completed up to 6 weeks or 42 days. During the puerperium, the reproductive organs will slowly experience changes like they were before pregnancy. This change in the reproductive organs is called involution. The postpartum period has several stages which will restore the condition of the mother to the time before pregnancy. There are changes experienced by postpartum mothers, both physical changes and psychological changes. During the postpartum period, it requires a visit to monitor the condition of the mother through the postpartum period at the first postpartum visit, which is 6-8 hours, the goal is to prevent bleeding during the puerperium, detect and other causes of bleeding, and provide counseling to the mother or family on how to prevent postpartum bleeding. at the second visit, which is 6 days after delivery, the goal is to ensure the mother's uterine involution is running normally, and ensure that the mother is breastfeeding properly and does not show any signs of complications, at the third visit, which is 2 weeks after delivery, the goal is to ensure that the mother's uterus is back normal, the fourth visit, which is 6 weeks after delivery, aims to provide early family planning counseling. Of the four visits, it can be assessed regarding the conditions experienced by postpartum mothers and by providing HE according to the conditions experienced by the mother at this time. Not only post-partum mothers, BBL also need assistance by health workers to monitor the condition of LBW, where the first three visits to BBL are made at the age of 6-48 hours, the goal is to find out abnormalities in babies as early as possible, especially in the first 24 hours of life. . The second visit to LBW, namely at the age of 3-7 days the goal is to keep the baby warm, early initiation of breastfeeding and at the third visit BBL at the age of 8-28 days the goal is to care for the umbilical cord and check the status of Vit K 1 prolaxis and immunization. supervision by health workers and an effort to reduce MMR and IMR. After all the postpartum period has passed, there are no problems and with low birth weight there are no problems, then the mother will get counseling about family planning in which family planning itself is contraception, which means preventing the meeting of eggs and sperm cells so as not to result in pregnancy. During this family planning visit, one visit was carried out and in conjunction with the four postpartum visits to provide counseling on family planning, what kinds of family planning and family planning are suitable for mothers to use immediately after the postpartum period.
