ASUHAN KEPERAWATAN KERUSAKAN INTEGRITAS KULIT PADA PASIEN DIABETES MELLITUS DENGAN LUKA GANGREN DI RSUD DR. SOETOMO SURABAYA
Abstract
Diabetes mellitus (DM) is a progressive chronic disease where the body
is unable to metabolize fats, proteins and carbohydrates that lead to
hyperglycemia (high blood glucose levels) and chronic complications of Diabetes
Mellitus which often occurs due to abnormalities in nerves, blood vessels and the
presence of infections that cause injury. Damage to the integrity of the skin tissue
is a condition where an individual experiences or at risk of damage to the
epidermis and dermis tissue or tissue of the mucous membranes, cornea, fascia,
muscles, tendons, bones, cartilages, joint capsules and ligaments. The objective of
the study is to carry out nursing care damage to skin integrity for patients with
Diabetes Mellitus with gangrene in Dr. Soetomo Hospital Surabaya. The method
used in this study is descriptive methods using case study approach. Nursing care
was carried out for 3x24 hours from December 27 to 29, 2019. The results
obtained within 3 days of implementation of the problem were resolved. It shown
the healing process of wounds, no smell, no pus, reduced tissue necrosis and the
granulation tissue appeared. The client has shown changes in wound area, 4 cm
wound wide, 1 cm deep, granulation tissue is growing well, and adheres to the diet
provided by the medical team. It is expected that hospitals can provide optimal
health services, especially for patients with Diabetes Mellitus.
