Abstract
One of the mental disorders is schizophrenia. Schizophrenia is a severe and
continuous neurological brain disease which can disrupt the lives of individuals,
families, communities, and can also influence client perceptions, ways of
thinking, language, emotions, and social behavior. Social isolation is a negative
symptom experienced by schizophrenic clients characterized by withdrawing from
the environment, preoccupied with his own thoughts, feeling insecure elsewhere,
unable to make decisions, unable to feel pleasure, and closeness with others. The
general objective of this case study is to provide client nursing care with social
isolation in the case of schizophrenia at the Wates Health Center. With a
descriptive method in the form of case studies with nursing care techniques that
include assessment, diagnosis, intervention, evaluation and nursing
documentation. Nursing care is conducted 19 meetings starting from 30
November 2019 until February 26, 2020. With 2 clients who have different
causes, client 1 requires 8 client meetings 1 faster than client 2 because the client
can get socially in touch with others quickly , supported by the existence of a
family that always supports client development in contrast to client 2 who are still
afraid and ashamed to have social relations in stages with other people, the family
lacks support with clients in every development and also the client's desire to be
able to return to social relations with the community does not exist.
