Continuous midwifery care, namely care that is carried out from the postpartum period, newborns, to family planning. This care aims to assist and monitor early signs of danger or abnormalities during the postpartum period, newborns and family planning. The method used in midwifery care is continuity of care which is carried out by home visits. Care for Mrs. "I" from the post-partum period, newborns to family planning by making 4 post-natal visits, namely the first visit at 3 days post SC, the second visit at 8 days post SC, the third visit at 2 weeks post SC and the fourth at 6 weeks post SC. There were 3 visits for newborn babies, namely at one visit 3 days after birth, the second visit at 8 days after birth, and the third visit at 2 weeks after birth, and one family planning visit that coincided with the fourth post-SC visit. From the results of the midwifery care for Mrs. "I" starting from the postpartum period, the newborns and family planning that had been carried out were not found any gaps in the condition of the mother and the baby, both of which did not experience complications or abnormalities between the facts and theories used with the condition of the mother and baby in normal conditions. Based on the implementation of continuity of care midwifery, there are several things and suggestions in an effort to improve the quality of services in reducing maternal and infant mortality rates for participants, health workers / midwives and for educational institutions.en_US
