    Abstract
    Maternal and infant mortality rates in Indonesia are the most important health problems and must be addressed immediately, because reducing MMR and IMR is an indicator of the success of a health service. The coverage of postpartum visits to family planning in Indonesia in 2018 was 85.92% while in 2017 the coverage of postpartum in Indonesia was 87.36%. To overcome this problem, the authors provide continuous and sustainable care, namely continuity of care (COC) by accompanying and monitoring from the postpartum period, neonates to mothers who become family planning acceptors. The care given during the postpartum period was 4 times, neonates 3 times and family planning (KB) service 2 times and was documented using the SOAP method. The study was conducted by collecting subjective and objective data from participants so that the data obtained from interviews with participants could be supported by examinations conducted by the author. To support data analysis, the authors also obtained data from the patient's MCH Handbook. The management of midwifery care for Mrs. R during the puerperium, a newborn, is in accordance with the existing theory. From the results of the research and provision of management through continuity of care by the author to the participants, the authors conclude that the care given to the participants greatly influences the health improvement of the participants. It is known that during the postpartum period, neonates to family planning participants do not experience a gap between theory and facts so that the postpartum period, neonates and KB activities take place normally.
