Abstract
The postpartum, neonatal, and family planning processes are basicallynatural processes, but in the process they can become complications that canendanger the mother and baby. The author aims to apply the method of continuousmidwifery care (continuity of care) at the Jatirejo Public Health Center, MojokertoRegency, this method is carried out through home visits and Via WA. Midwiferycare (continuity of care) is carried out using midwifery management and the
SOAP documentation method. The author made 4 postpartum visits, 3 neonates,
and 1 family planning visit.
There were 4 postpartum visits, namely 1 time at the Jatirejo Health
Center and 2 times at the patient's house 1 Via WA and the results werephysiological. Neonatal visits were carried out 3 times, once at the Jatirejo Health
Center, and 2 times at the patient's house, whose implementation schedule
coincided with postnatal visits, the result was no pathological condition. Thefamily planning visit is carried out once, namely on the 42nd postpartum day, thepostpartum mother is sure to use 3-month injection of family planning and plansafter this.
Assessment of subjective and objective data, preparation of midwiferydiagnoses, planning, management, evaluation of management, and documentationof midwifery care at Mrs. A during childbirth, neonates, and family planning arein accordance with the theory.
With the assistance provided, it can improve the welfare of mothers andbabies so that they can support government programs to reduce maternal mortality
(AKI) and infant mortality rates (AKB).
