dc.description.abstract | Continuity of care is the provision of midwifery care starting from the postpartum period,
neonates or newborns to deciding to use family planning, this is an effort to help monitor the
possibility of complications that accompany mothers and babies from the postpartum period to the
mother. using KB. The author aims to apply the method of continuous midwifery care (continuity
of care) at Jatirejo Public Health Center, Mojokerto Regency, this method is carried out through
home visits. The purpose of preparing this Final Project Report is to provide continuous midwifery
care (contuinity of care) and carry out SOAP documentation during the postpartum period and
family planning using midwifery care management.
Midwifery care for Mrs. N from the postpartum period, neonates to family planning.
Midwifery care during the postpartum period was carried out 4 times during the first postpartum
visit at the Jatirejo Public Health Center, Mojokerto Regency and subsequent visits were made at home visits. Three visits to newborn babies during the first visit were carried out at Jatirejo Public
Health Center, Mojokerto Regency and then a home visit was carried out. Family planning visits
are carried out once at home visits.
Mrs. N carried out Continuity of Care, namely from the results of the examination of
obstetric care in postpartum mothers who were carried out 4 times the results were physiological,
the neonates who had 3 visits did not find any problems that led to pathology and family planning
that had been carried out 1 visit also within the physiological limit. found problems that lead to
pathologies and complications so that the Continuity of Care reduces maternal and infant mortality
rates.
Of all the midwifery care that has been carried out, no complications have been found
and based on the implementation of continuity of care midwifery, it is hoped that midwives can
