The postpartum process, newborn babies, and family planning are basically a natural &#13;
process, but in the process it can become a problem or complic ation at any time that can &#13;
endanger both the mother and the baby. Ther efor e, the author aims to apply the method of  &#13;
continuous midwif ery car e (continuity of car e) at the Puskesmas Bangsal Mojokerto Regency,  &#13;
this method is carr ied out through home v isits.  &#13;
 Midwifery car e ( continuit y of care)  is carried out u sing midwif er y management and the &#13;
SOAP documentation method. Midwifer y care for Mrs. R P20002 was conducted on March 11,  &#13;
2020 at PMB Lilik Fauzah Amd. Keb Region Bangsal Community Health Center,  Mojokerto &#13;
Regency. The author condu cted 4 postpartum visits, 3 newborns at the same time and 1 f amily &#13;
plann ing visit.  &#13;
 Ther e were 4 postpartum visits, namely 1 time at PMB and 3 times at the patien t's &#13;
house and the r esults were physiological. Dur ing the visit, the newborn was carried out 3 times, 1 time at PMB and 2 times at the patient' s house whose implementation schedule coincided with &#13;
the postpar tum visit, the result was no pathological condition. The family planning visit was &#13;
carr ied out once, namely at the 7th week of the puerperium, the patient was inter ested in using &#13;
KB MAL. The r esults of midwifery care showed conformity to data and theor y in each &#13;
postpartum phase, n eonates, and family planning.  Each phase is going well because the condition &#13;
of the mother and b aby is good.  &#13;
 Management of midwif ery car e at Mrs. R during the postpartum period, newborn &#13;
babies, and f amily planning are in accord ance with the theory and it is hoped that midwives in &#13;
carr ying out con tinuity of car e can then apply midwifery management, maintain and be able to &#13;
incr ease competence in providing care in accordance with midwifer y service standards.

Continuity of care is the provision of midwifery care starting from the postpartum period, &#13;
neonates or newborns to deciding to use family planning, this is an effort to help monitor the &#13;
possibility of complications that accompany mothers and babies from the postpartum period to the &#13;
mother. using KB. The author aims to apply the method of continuous midwifery care (continuity &#13;
of care) at Jatirejo Public Health Center, Mojokerto Regency, this method is carried out through &#13;
home visits. The purpose of preparing this Final Project Report is to provide continuous midwifery &#13;
care (contuinity of care) and carry out SOAP documentation during the postpartum period and &#13;
family planning using midwifery care management. &#13;
 Midwifery care for Mrs. N from the postpartum period, neonates to family planning. &#13;
Midwifery care during the postpartum period was carried out 4 times during the first postpartum &#13;
visit at the Jatirejo Public Health Center, Mojokerto Regency and subsequent visits were made at home visits. Three visits to newborn babies during the first visit were carried out at Jatirejo Public &#13;
Health Center, Mojokerto Regency and then a home visit was carried out. Family planning visits &#13;
are carried out once at home visits. &#13;
 Mrs. N carried out Continuity of Care, namely from the results of the examination of &#13;
obstetric care in postpartum mothers who were carried out 4 times the results were physiological, &#13;
the neonates who had 3 visits did not find any problems that led to pathology and family planning &#13;
that had been carried out 1 visit also within the physiological limit. found problems that lead to &#13;
pathologies and complications so that the Continuity of Care reduces maternal and infant mortality &#13;
rates. &#13;
 Of all the midwifery care that has been carried out, no complications have been found &#13;
and based on the implementation of continuity of care midwifery, it is hoped that midwives can &#13;
maintain the quality of health services.

The provision of coninuty of care that will be carried out by midwives&#13;
on an ongoing basis for postpartum mothers, newborns and family planning&#13;
acceptors, with the aim of reducing high risks and those that will cause&#13;
complications leading to death in mothers and babies. The author provides&#13;
assistance to Mrs. Y, 35 years old by visiting during the postpartum period,&#13;
neonates, and family planning and providing care according to the needs of the&#13;
mother and baby.&#13;
 Care for Mrs. Y was given from 9 March 2020 to 20 April 2020 as many&#13;
as 8 visits, namely 4 postpartum visits, 3 times neonatal visits, and 1x family&#13;
planning visit. The results of the postpartum visit, the mother complained of pain&#13;
in the suture wound at 6 hours post partum. The complaints felt by the mother are&#13;
within physiological limits. At the neonatal visit there were no complaints, the&#13;
baby was in good health and physiological condition. As well as during the family&#13;
planning visit, the mother agreed to use 3-month injection contraception.&#13;
 The care that has been given can provide treatment and be applied.&#13;
Mothers are cooperative in examining and receiving health education properly, so&#13;
that the authors can provide care according to the needs of mothers and babies.&#13;
 It is hoped that with the continuous midwifery care during the&#13;
postpartum period, neonates and family planning can reduce high risks and that&#13;
will cause AKI and IMR to increase and support women to make decisions about&#13;
the health of mothers and their babies.

 Care for Mrs. M was given from 18 March 2020 to 27 April 2020 as&#13;
many as 8 visits, namely 4 postpartum visits, 3 times neonatal visits, and 1x&#13;
family planning visit. As a result, from the post-partum visit, the mother&#13;
complained of pain originating from the post-SC scar. The complaints felt by the&#13;
mother are within physiological limits. At the neonatal visit there were no&#13;
complaints, the baby was in good health and physiological condition. As well as&#13;
during the family planning visit, the mother agreed to use 3-month injection&#13;
contraception.&#13;
 The care that has been given can be understood and applied. Mothers are&#13;
cooperative in examinations and receive good health education, so that the authors&#13;
can provide care according to the needs of mothers and babies.&#13;
By providing care for postpartum mothers during a pandemic like this, the authors&#13;
found few limitations in knowing the health development of their participants,&#13;
who usually carried out direct examinations and used objective data, but during&#13;
this pandemic the authors only obtained data subjectively from the participants. It&#13;
is hoped that the existence of continuous midwifery care during the postpartum&#13;
period, neonates and family planning can reduce high risks and that will cause&#13;
AKI and IMR to increase and allow women to participate in making decisions&#13;
about the health of mothers and their babies.
