dc.description.abstract Surgery was one of the medical procedures that triggered psychological responses in the form of preoperative anxiety. Untreated anxiety affected hemodynamic stability and increased the risk of perioperative complications. Music therapy was selected because it was a safe, non-invasive, and easy-to-implement non-pharmacological nursing intervention that had been shown to promote relaxation and reduce anxiety. This study used a descriptive case study design with a nursing care process approach involving one preoperative patient with a femoral fracture and anxiety at RSI Sakinah Mojokerto. Primary data were collected through interviews, observations, physical examinations, and anxiety assessment using the Hamilton Anxiety Rating Scale (HARS), while secondary data were obtained from medical records and nursing observation notes. Instrumental music therapy was administered for 15 minutes daily over three consecutive days according to the Indonesian Nursing Intervention Standards (SIKI). Data were analyzed descriptively by comparing the patient's condition before and after the intervention. Before the intervention, the patient experienced moderate anxiety with a HARS score of 25, accompanied by restlessness, insomnia, palpitations, a blood pressure of 150/95 mmHg, and a pulse rate of 104 beats/minute. After the intervention, the anxiety level decreased to mild anxiety with a HARS score of 12. Blood pressure decreased to 132/80 mmHg, pulse rate decreased to 82 beats/minute, and the patient appeared calmer with improved sleep quality. Music therapy proved effective in reducing anxiety and improving physiological responses. The findings suggested that music therapy could be applied as a simple, safe, and holistic non-pharmacological nursing intervention for preoperative patients with anxiety. en_US
