dc.description.abstract Patient with chronic kidney failure almost all have skin rash disorders, the most common of which arise including prunitus or dry skin itching, and scaly, to rough and cracked. If not treated will cause serious problems, the skin will be damaged and lose moisture. As a result, there will be damage to the integrity of the skin. Damage to skin integrity is damage to the skin(dermis and/or epidermis) or tissues. The purpose of this study was to carry out nursing care with the problem of skin integrity damage to clients with chronic kidney failure at Gatoel Mojokerto Hospital. The design carried out in this research is a case study, with the method of interview, observation and documentation study. The result showed that both participants had the same skin conditions, namely dry and itchy skin. Both clients are given the same action. The focus of the action taken is giving virgin coconut oil or VCO on the client’s dry and itchy skin. Based on the research, it was found that results of the evaluation of the problem of skin integrity damage were resolved, as evidenced by the fact that both of them had reached the 4, predetermined outcome criteria. From this study it can be concluded that patients with chronic kidney failure with skin integrity damage are very important to maintain and provide moisture to the skin. In this case can also use virgin coconut oil or VCO. en_US
